Agency Depopulation Round 2 Registration Screen

This initial registration will allow Louisiana Citizens to provide your agency with a secure login and detailed instructions for accessing and using the depopulation portal, beginning October 1, 2008.

The following information is required to complete the registration process and be able to participate in the round 2:

1. Agency Code Number (last four digits)
2. Name of the Owner/Manager (list one)
3. Representative who will be responsible for the program (could be same as owner)
4. Address
5. Phone number
6. Fax number
7. E-mail address

Your four digit agency code will allow you to log into the registration system. Please review provided information; update as necessary, and complete all blank fields. If the owner/manager and representative are the same please select same.

Louisiana Citizens will communicate information for Depopulation Round 2 using the representative’s email address.
