If you are a new agency and are applying for access to Louisiana Citizens products and systems for the first time, please complete the form below. You will also need submit the following documents via email to policyadmin@lacitizens.com or fax to (504) 831-6676. Once this information has been received and approved, you will receive an email with your LA Citizens Producer Code and Login information:

• Completed W-9 form for agent or agency
• Copy of Property/Casualty License
• Copy of Errors & Omissions Insurance
